[ic] Creating new forms

Hi;

I just need some help with new form creations using interchange.

I need to create new form where users will be able to submit some
comments about our webpage. All informations entered by user (name,
address, etc) need to be saved to database.

Could you please help me and point me somewhere so I can find out the
easy way of doing it.

This may be silly question, but I am completely new in this and need
help.

So please help me.
