[ic] error adding to transaction log

I just started having a new problem.

It seems that when you click Place Order in the checkout page, it just loops
back to the checkout page again. No red text to indicate missing fields and
no error messages except in the log files.

I suspected a missing or corrupt file, so I had the ISP delete my entire
site and re-install. then restored from a known working backup.

Any help is appreciated.
