[ic] Affiliate Login Page Problem

Hi,

I've tried to research the archives to find an answer to a problem I'm 
having with the affiliate login page.  For some reason, I get just 
the "Log in as affiliate" sentence at the top of the page, but not the 
associated login/password boxes.  When looking at the html, the form 
for the boxes is there, but it just doesn't show on the page.  I saw 
the problem listed several times in the archives, but I never found a 
solution.  Can someone help??
