We use dedicated boxen at Inflow.com for some of our clients, but that's
several grand/mo.

Dialtone.com has dedicated Linux boxen for $99/mo.
Rackspace.com has dedicated Linux boxen for $200/mo.
Hostpro.com (now Interland) has a shared-server with root access available
for $70/mo (BSD, though).

If your looking for something cheaper, here is a small compilation of
hosting providers that have been known to run IC daemons (no particular order):

websitecreations.net
interserver.net
rackhost.net
hostroute.com
pair.com
vervehosting.com
123ehost.com
digitalomaha.net

And then there are the people on this mailing list who have mentioned they
(or their employers) run a hosting service (in no particular order):

namisolutions.com
lathi.net
newmediaems.com
korksoft.com
addaction.net
hcst.net
adgrafix.com
digilink.net

I hope that helps.
