This is probably due to UPS Ground having been previously saved as the 
default shipmode for that account in userdb. Did you try changing the 
shipmode to FREE on the account maintenance page, saving the change 
(clicking Update), then logging out / logging in?

You may want to verify that you have this in your catalog.cfg

ValuesDefault   country      __SHIP_DEFAULT_COUNTRY__
ValuesDefault   mv_shipmode  __SHIP_DEFAULT_MODE__

and that SHIP_DEFAULT_COUNTRY and SHIP_DEFAULT_MODE are set to US and FREE, 
respectively.
