Mike,

Login to admin and go to 'design'. Adjust the section properties to your
liking then hit 'auto-populate' (you may have missed this link because
its somewhat camouflaged against the grey section header. This will
rebuild categories and product groups using [only] the ones currently in
your products database...

ive been using the table editor to add categories and then add them in
the
wizard. its not exactly what you need to know but its saved me a fair
amount
of time in building my menus.
