FLAT  Flat Rate    price     0       0       e Nothing to ship!
FLAT  Flat Rate    price     0.01    20.00   f 3.99
FLAT  Flat Rate    price     20.01   30.00   f 4.99
FLAT  Flat Rate    price     30.01   40.00   f 5.99
FLAT  Flat Rate    price     40.01   50.00   f 6.99
