This one is only for the terminal.

And ensure that 'mv_order_profile' is set to 'credit_card' if it's not done
automatically via the choice of payment method.

Four places, as mentioned under "troubleshooting". If you run 
Interchange in the foreground you'll see it all on screen as it 
happens. If you don't see it, it's not happening, but it definitely
should with these settings.
