[ic] Options setup

I want to setup some options, but I'm a bit confused how to setup.  The
product is basically as follows:

Chin Rests

Description          Plastic     Ebony     Rosewood    Boxwood
-----------------  --------  --------  ------------- ----------
Teka                    4.50       15.00     15.00            18.50
Berber                 -             15.00     15.00            18.50
Shmidt                 -             15.00     15.00            18.50

There is also an option of whether the Chin Rest is for Violin or Viola.

What would be the best way of setting this up?
