I double checked that and, yes, I do hit the "OK" before changing tabs. 
It looks like the tabbed input is not stored at all and each tab must be
saved before going to the next. Which is sort of broken as the "OK"
button does not return you back to the tab, but the preceding page.

The image file is _still_ not uploaded into the directory structure.

I am suspicious now that there is a setting wrong in the web server.
