[ic] Checkout Calculation / Missing Card Form

Hi,

I've come across the same problem on three different catalogs now build on
foundation 5.x, which is that when a user goes to check out, the inital
payment checkout page as presented doesn't have the shipping charge in
place, has "Credit Card" selected as the payment type but the card form is
missing saying "No payment method is selected."

The user can hit 'Recalculate' to get around this problem, however it
shouldn't be necessary for the user to do that.  Any suggestions how to
ensure that when the user goes to checkout, the shipping charge is
automatically calculated according to the default ship mode, the payment
method is set to "Credit Card" and the card payment form is visible?
