It looks like some sort of bot bug to me.  The IP looks genuine.  The
request would have been harmless as far as Interchange was concerned.

I don't know whether you trimmed the log message or whether your
Interchange version is just old, but you should have found the following
in your global error.log file:

    Security violation -- scripting character in page name
    '+ tprepend + l[ top_PAGE ] +'.

Interchange saw the "[" character in the URI and refused to handle the
request.

If you had a violation page then that would have been returned.  In your
case, an "internal server error" message would have been returned instead.
