Thanks Curt. I ended up figuring out a solution that I think will work.
I created a shipping method called "default" and set that as the default
(of course). It's a method based on the number of items in the cart and
displays a message "Please select a shipping method".
I then added a regex check to my order profiles to return an error if
mv_shipmode is set to "default" when an order is placed. Seems to be
working so far.
