I also had this same problem a few weeks ago.  The docs appear to be in 
error.  Replace these lines:

<INPUT TYPE=hidden  NAME="mv_order_group"  VALUE="0">

with this line:

<INPUT TYPE=hidden  NAME="mv_order_group"  VALUE="1">
