[ic] Foundation simple option text area widget

mv_order_Distance: Need to select Distance to wall at center  <---
mv_order_flat_width: Need to select Flat width edge to edge  <---

This is for leaving two text area boxes blank in a simple option. Most
select boxes left unselected is acceptable to a customer, but a comment
of info text box confuses the customer if they do not have a comment. I
change the widget from a select box to a text box in simple options.
